Changing Gender on Your Indiana Birth Certificate
(Information verified as of 2006) – This applies to individuals born in the state of Indiana. Regardless of which part of the state you were born in, there is a copy of your birth record with the Department of Vital Statistics within the Indiana State Department of Health located in downtown Indianapolis, Indiana.
In Indiana, there are two forms of birth certificate that you can get based on your birth record. One is the short form, an 8.5″ by 5.5″ version which just shows the basic information (name, father, mother, date, etc.). The other is the long form, which is your entire detailed record. In the past, Indiana did not show gender on the short form version. That has now changed. The short form and the long form, if issued now, would show the gender that was entered on the original record when you were born.
You can change the gender on your birth record by getting a court order. To change the gender on your birth record, do the following:
- document to a court in the county where you now live that you have had sex-change surgery,
- request a court order that will order the Indiana State Department of Health (ISDH) to change the sex on your birth certificate,
- send a copy of the order to the ISDH at the address below along with an $18.00 check or money order made out to “ISDH.”
